Because of the many options offered within our products, our order system is NOT automated. How To Order - SECURE ORDER FORM Option 1: Use the Secure Online ORDER FORM Open it up by CLICKING HERE. YOU NEED TO HAVE YOUR POP-UP BLOCKER TURNED OFF IN ORDER FOR THE ORDER FORM TO COME UP AS A SEPARATE WINDOW. Locate the items that you are interested in within this web site and simply type in the form fields with the item number, item description, options (if offered), quantity, and cost (you can also copy and paste). Include sales tax (for IL shipments only) and shipping costs (see chart to the right). Or we can calculate the tax and shipping for you. When your order is complete, click on "Submit" to execute the order. If you prefer not to include your credit card information, that is fine. We can call you for your credit card information or we will wait for your check to arrive (checks will need to clear before we can process your order). Please indicate if you will be paying by check or credit card. You may also print this form out and fax it to us at 708-445-7101. Option 2: Download a .PDF version of ORDER FORM Print a copy of this form.
